- Place of Manufacture: Full factory name, complete factory address, and valid contact info
- Product Test Date: Date of the latest official compliance test
- Testing Laboratory Details: Full name, address, contact information of the test lab. Note: All children’s product tests must be completed by a CPSC-recognized certified laboratory (verifiable via cpsc.gov/LabSearch).
- Point of Contact: Full name, address, phone number, and active email of the compliance record keeper.
